After Service: How DVA-Funded OT and Social Work Help Veterans Live Well at Home

Serving in the military demands everything from your body and your mind. But the support you need after service, to live well, stay independent, and feel connected, often isn’t as visible as it should be.

At Proactive Health, our Occupational Therapists and Social Workers work specifically with veterans and their families to make sure the years after service are supported, practical, and dignified.

And for eligible DVA Gold and White Card holders, many of these services are available at no out-of-pocket cost.

Falls are the leading cause of injury hospitalisation for older Australians. For veterans managing chronic pain, reduced mobility, or neurological conditions from service, the risk is even higher.

Our OTs conduct thorough home safety assessments, identifying hazards and recommending practical modifications that make a real difference:

  • Grab rails in bathrooms and hallways
  • Ramps and step modifications at entries
  • Improved lighting and trip hazard removal
  • Non-slip surfaces and bathroom safety equipment

You’ve earned the right to feel safe in your own home. We help make that happen.

“Struggling to Get Out of Bed or Off the Chair?”

Reduced strength, joint pain, and mobility changes can make everyday tasks that were once effortless feel like a real effort. Our OTs help veterans access the right assistive equipment to restore independence and reduce strain:

  • Walking aids and mobility equipment
  • Electric recliners and adjustable beds
  • Transfer aids and slide sheets
  • Reaching and gripping aids for daily tasks

Where equipment is DVA-funded, we manage the paperwork and procurement process so you don’t have to.

“Finding Cleaning or Gardening Harder Than It Used to Be?”

Maintaining your home and garden matters, not just practically, but for your wellbeing and sense of independence. Our OTs assess your functional capacity and link you to DVA-funded home services including:

  • Domestic assistance and cleaning support
  • Garden maintenance services
  • Meal preparation support

You don’t have to do less. You just need the right support in place.

“Recently Discharged from Hospital and Struggling at Home?”

Post-hospital recovery is one of the most vulnerable periods for veterans and older Australians. Our OTs can organise a rapid response to ensure your home is set up for safe recovery, with equipment delivered, modifications in place, and a plan for the weeks ahead.


Social Work for Veterans and Everyone Else

Life throws challenges at all of us. Grief, relationship breakdown, financial stress, mental health struggles, isolation, and transitions that feel overwhelming. You don’t have to navigate them alone.

At Proactive Health, our Social Workers provide friendly, practical, and personalised support that fits your unique situation. Whether you’re a veteran, an NDIS participant, or simply someone going through a hard time, our Social Workers meet you where you are.

We support people to:

  • Manage mental health and emotional wellbeing, with practical coping strategies, connection to services, and someone who genuinely listens
  • Stay connected with community, reducing isolation, building social supports, and linking you to local groups and networks
  • Navigate practical life challenges including finances, employment, housing, and accessing entitlements you may not know about
  • Move through change, whether that’s a health diagnosis, a loss, a life transition, or a new beginning

For veterans specifically, our Keep Connected program focuses on rebuilding social connection after service, addressing isolation, and creating meaningful engagement with community and like-minded people.

Our approach is non-judgemental, strengths-based, and focused on empowering you, not creating dependence.

Access OT and Social Work Through Proactive Health

Veterans: DVA Gold Card covers OT services for eligible conditions. White Card covers service-related conditions. Ask us about what you’re entitled to. Many veterans are surprised by the scope of support available.

NDIS participants: OT is funded under Capacity Building (Improved Daily Living) and Capital Supports (Assistive Technology and Home Modifications). Social Work may be funded under Improved Daily Living.

General community: Medicare GPMP/TCA referrals, private health, and self-referral options available.
