Ongoing Review & Empowerment

Regular multidisciplinary reviews ensure your medication, nutrition, and exercise plans remain aligned and effective. The program transitions toward self-management, equipping you with the knowledge, habits, and confidence to maintain your results.
Who is it for

Is Find Your Form Right for You?

  • BMI of 30 or above (or 27+ with weight-related health conditions)
  • Tried diet and exercise alone without lasting results.
  • Related health conditions such as type 2 diabetes, cardiovascular disease, sleep apnoea, or metabolic syndrome.
  • Want a structured, team-based approach rather than navigating weight management alone.
  • Seeking sustainable, long-term change — not a quick fix.
Eligibility for medication-assisted weight management is determined during the initial medical consultation. Not all participants will be prescribed medication.
