As qualified professionals, social workers have the skills and knowledge to support individuals, families, groups and communities and partner with people to address personal difficulties and structural barriers in their lives.

Your Proactive social worker will guide you through your allied health sessions and play a key role within the allied health network in assisting clients transition through their health journeys, whilst interacting with a wide range of other health professionals based at the clinics.

The first session is an opportunity for the social worker to gain an understanding of the support you require. They may ask a number of questions about your history/background and current circumstances, including work, family and relationships, housing, and physical and mental health. As your sessions continue, your social worker will look at all aspects of your life and partner with you to discuss possible solutions, supports and pathways. 

Our social workers deliver support on a one-on-one basis and also via group sessions. Proactive social workers are trained to listen to people and support them to address personal difficulties and structural barriers in their lives. They offer a choice between face-to-face sessions and telehealth appointments to provide the best quality care to their patients. 

Creating a wraparound service to support our clients every step of the way through their health treatment and care plans, linking in with one of our social workers will be one of the best health care choices you make.
